Epstein’s cellmate was released the day before his death. On the night of 9 August the prison guards also failed to conduct checks scheduled for 03:00 and 05:00, prison documents show, and the camera system in the unit was also down. His body was discovered during a morning check carried out by staff.
A second, redacted version of the same FBI report that is only 17 pages long has also been published as part of the Epstein files. It does not include the psychologist’s report or the timeline of his detention and the images in the file are redacted. It is not clear why both redacted and unredacted versions of the report have been included in the files.
The DoJ has been contacted for comment. The FBI declined to comment.

Cell where Jeffrey Epstein died never underwent proper inspection, experts say
The Epstein Files now show the world that Ghislaine Maxwell knew about all the inhumane atrocities and was actively involved.
Ghislaine Maxwell is currently serving her 20-year sentence at the Federal Prison Camp in Bryan, Texas.
I cannot rule out the possibility that Ghislaine Maxwell, in her prison where she can move freely and is not allowed to be contacted by other inmates, might one day disappear like Jeffrey Epstein.
I cannot rule out murder.
Based on reports, at least three prominent accusers/victims of Jeffrey Epstein have died since the scandal intensified, along with several key associates.
In April 2025, it was reported that Virginia Giuffre, a prominent accuser who alleged she was trafficked to Prince Andrew and others, died by suicide in Australia.
Carolyn Andriano: Identified as a victim, she passed away in May 2023 at age 36.
Leigh Skye Patrick: Identified as a victim, she passed away in May 2017 at age 29.
